Assessments

When you meet with an Aging Life Care Professional, you can count on someone to listen to you, assess your current strengths and needs, and develop a care plan centered around you or your loved one. While everyone’s needs may be different, assessments will evaluate any or all:

Level of current family, social, and community supports

Current and history of physical, cognitive, and emotional health

Level of safety requirements and available supports

Financial resources and needs for advocacy.
