About Entrusted Care


Entrusted Care was founded after working with our own and other families struggling to access services and coordinate many moving parts for our parents in need and for the family as well.  Communicating with and navigating our parents’ myriad of health appointments, ensuring the family’s understanding, compliance with medications, follow-up, arranging consistent care givers, and ensuring comprehensive and consistent care was a challenging and orchestrated juggling act.   Family communication demands, our own immediate family and work responsibilities, distance, and especially ever changing care givers from well-intentioned nursing agencies, increased family stress levels at a time when fear and concern for the parent’s quality of life were at its height.  We knew this struggle was not unique to our family and so grew a deep desire to provide continuity, support, and peace of mind for other families in similar situations.  Entrusted Care will be that continuity.  We will be that main contact and Aging Life Care Professional whose job it will be to coordinate services amongst physicians, nursing agencies, care givers and your family.  While things in yours or your loved one’s life may be changing, we will not.  We are here to be entrusted with coordinating and communicating consistent care.


Entrusted Care Founders combine over 20 years of healthcare and social work experience as well as financial and industry experience.


Gina Donnelly is an experienced and educated Social Worker with solid Health Care experience in Acute Care and Nursing Home Care Coordination as well as working in social service settings.


Kathy Jacobs is a proven leader in project management, logistics, and financial services.  In addition to holding degrees in both Psychology and an MBA from the Simon School of Business, Kathy possesses experience in working with people with disabilities and those with chronic/terminal illness.
