Entrusted Care seeks to bring calmness and clarity to elders
and their families at a time when confusion, uncertainty and
fear are prevailing in their lives. We have personally
experienced the challenges a family faces when caring for
aging and/or ill parents and family members. Even with a
wealth of industry experience, the situation can be
emotionally draining.
We want families to enjoy true quality time with their aging
loved ones so we offer our knowledge and experience to
assist with the management and coordination of their care.
Aging Life Care™, also known as geriatric care management,
is a holistic, client-centered approach to caring for older adults
or others facing ongoing health challenges. Working with families, the expertise of Aging Life Care Professionals provides answers at a time of uncertainty. Compassionate guidance leads families to the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time off of work for family caregivers.
Entrusted Care Founders combine over 20 years of healthcare and social work experience as well as financial and industry experience.
Gina Donnelly is an experienced and educated Social Worker with broad Health Care experience in Acute Care and Nursing Home Care Coordination as well as working in social service settings. She has worked in discharge planning, admissions, case management and other human services roles giving her a knowledge base that truly serves the senior community. Gina is is a Care Manager Certified by National Academy of Certified Care Managers and is recognized as an Advanced Professional by the Aging Life Care Association.
Kathy Jacobs is a proven leader in project management, logistics, and financial services.
In addition to holding a Bachelor's degree in Psychology and an MBA from the Simon School
of Business focused on Health Care Operations, Kathy possesses experience in working with
people with disabilities and those with chronic/terminal illnesses. Kathy also holds a
Gerontology Certificate from St. John Fisher College and is a Professional Member
with the Aging Life Care Association.
Gina and Kathy are both members of the National Aging in Place Council and are founding members and officers of the Rochester/Finger Lakes Chapter of the NAIPC. They are also Certified Aging in Place Specialists as designated by the National Association of Home Builders.
